RN - Geropsychiatric Unit Experienced RN (minimum one year) to work with adult and geriatric patients; current BCLS and AB 508 training required. Responsibilities include: • Assessing patient and family needs and completing admission assessment documentation. • Initiating Patient treatment Plan form assessment information which includes nursing diagnoses, goals and interventions. • Documenting patient status at the time of admission, transfer or discharge to and from nursing unit. • Setting priorities in planning for patient needs to include patient and/or family teaching. • Communicating with and utilizing available resources within the facility and community in planning all aspects of nursing care.
Dept: Geropsych	Status: Per Diem/On Call	Hours: Needed for both Day and Night shifts

Quality Management Coordinator - RN Reporting to the Director Quality and Risk Management, is responsible for planning, implementing and managing the Hospital's Quality Management Program for all departments in compliance with State, Federal and Medicare regulations. Requires current RN license, Bachelor’s preferred; BCLS required; recent experience in Quality Management in an acute-care setting preferred; good communication, presentation and computer skills. CPHQ Preferred.

RN-Transitional Care Unit RN license, previous experience with adult and geriatric patients, minimum 2 year experience in a SNF or similar facility, current BCLS. Our TCU (Transitional Care Unit) is a high acuity hospital-based Skilled Nursing Facility with short-term stays.
