Our Team V-Care Wellness Plan is designed to help you achieve and/or maintain a healthy lifestyle while saving you money! Full participation in this program is completely voluntary but it does allow you to receive a significantly lower medical insurance premium.
Health Evaluation – The Plan’s free health evaluation consists of a blood screening, blood pressure analysis, PSA test (males 40+), thyroid test (females 40+) and web-based health risk assessment. These diagnostic tests can detect a wide range of health problems including cardiovascular disease risks (high cholesterol, high blood pressure, and diabetes), liver and kidney disease, anemia, and certain types of cancer. Based on your test scores, you will receive a confidential personalized health information report that allows you to better assess your risk for heart attacks, strokes, mental health problems, and sleep disorders.
Lifestyle Management Programs – Team V-Care health educators and lifestyle management programs are available to all plan participants and their covered dependents. A health educator collaborates with you to make lifestyle changes so you can live a healthier life. They are also available to answer your health related questions, assist you with a newly diagnosed condition, or work with you on a lifestyle management program. These programs include:
Disease Management Program – Team V-Care can also provide a confidential program to care for chronic health conditions, such as asthma, COPD, diabetes and heart disease. Team V-Care nurses and health educators can help you and your family better understand your illness and what it means to your lifestyle. They will work with you and your doctor to insure you understand your care.
